CHINA TRIP COMMITMENT FORM

_____ My/Our son/daughter WILL PARTICIPATE in the proposed trip to China by the New Trier Chamber
Orchestra and Jazz Ensemble during the summer of 2000, should the trip take place. I/WE understand that the
trip cost is $2,100 and assume responsibility for that cost, and the additional personal expenses associated
with such a trip. (Refer to the Tour Proposal.) We are aware of the schedule of payments and due dates
and will abide by that schedule. A $250 initial deposit accompanies this form.
I/We understand that our son/daughter will be given the opportunity to raise funds that either will
directly result in the lowering of his/her specific trip cost, or will result in the lowering of costs for
the entire tour party. All funds will be deposited in a China Tour account maintained by the New Trier
Business Office and fund raising benefits will be subtracted from the final trip
payment(s). All tour
payments will be made payable to New Trier High School.
I/We have received the rules to be in force during this trip and agree to abide by those rules
(“Tour Expectations”). We are particularly aware of the
policy that my son/daughter may either be restricted from
performance(s) or be sent home at my/our expense
on the next available airplane if my son/daughter commits a gross violation of the tour rules, and that
on behalf of New Trier High School, Peter Rosheger and/or James Warrick has been empowered to make
decisions regarding the disciplinary procedures during this trip in accordance with those policies. We
understand that there will be a specific “no tolerance” policy specifically regarding the use of
unauthorized drugs or alcohol.
I/We understand that our son/daughter will be required to attend occasional forums at the school for the
purpose of educating our son/daughter about the customs, history, language, or other unique aspects of
China. These meetings may or may not take place during class time.
I/We understand that there will be additional forms relating to medical, disciplinary concerns, or
liability issues to be signed before the trip
commences.
